Story County - Community Health Needs Assessment
Every 5 years, county public health agencies across the state conduct a formalized Community Health Needs Assessment and create an associated Health Improvement Plan. The assessments are carried out in a collaborative nature, between Story County Public Health and the Story County Quality of Life Alliance and its members.
The 2015 assessment was comprised of several components, including a survey of SCQLA member agencies and other partner service agencies in the community. This data was combined with various external data resources including:
County Health Rankings from Robert Woods Johnson Foundation,
CDC's Community Health Status Indicators,
Story County reports from Community Commons.
From our most recent 2015 Health Needs Assessment, the top priorities in our community are:
1. Hunger
2. Access to mental health and crisis services
3. Exercise and physical activity, geographically focused in greater Story County
The data obtained from the report provide a valuable resource for understanding our local needs while also serving as a benchmark to track progress over time. Additionally, SCQLA has placed particular emphasis on standardizing efforts to address identified needs. This is to ensure transparency in efforts and promote community dialogue around these topics.
